Player Waiver & Release Form
Before participating in any activity at Mana Sports Club, every player must complete and sign this waiver form.
By signing, you acknowledge that sports and recreational activities involve certain risks, and you agree to participate responsibly. This form helps us keep all guests safe and ensures that everyone enjoys their time in a fun, respectful, and secure environment.
If the participant is under 18 years old, a parent or legal guardian must complete and sign the waiver on their behalf.
Once submitted, your waiver will remain valid for one year from the date of signing.
Waiver Renewal
If it’s been a year since you last signed your waiver, it’s time to renew it to stay active and ready to play at Mana Sports Club.
Each waiver remains valid for 12 months from the date of signing. To continue participating in our activities, leagues, and events, please complete the renewal form below.
